You can choose to turn any of these forms on or off at any time.

The information entered by your patients is kept in your secure management logs and a notification of a new request is sent to an e-mail address or e-mail addresses of your choice. Then simply login to your management area to view your logs. You can create as many management user accounts for your logs as you need.

These forms could reduce the number of trips your patients need to make to the surgery.
